  • Monday through Friday schedule, finishing by 3pm daily
  • Patient load of 3 – 9 patients per day with extended 1:1 time
  • Assisted living and skilled nursing facility rounds as needed
  • Team-based care with interdisciplinary meetings twice weekly
  • 1:4 call schedule with RN triage support — primarily telephone
  • Hospital and long-term care rehab rounding within 1-hour radius
