  • VA outpatient primary care clinic
  • Member of VA Patient Aligned Care Team (PACT)
  • Chronic disease management — hypertension, diabetes, CAD, PTSD, depression
  • Preventive care including physicals, screenings, immunizations, wellness visits
  • Acute care — respiratory illness, wounds, dermatology, ENT, GI conditions
  • Minimum 2 years of recent primary care experience required
